ACCREDITED PROVIDER PLANNING TEMPLATE

The Nurse Planner must be a registered nurse who holds a current, unrestricted nursing license (or international equivalent) AND hold a baccalaureate degree or higher in nursing (or international equivalent) AND be actively involved in planning, implementing, and evaluating this NCPD educational activity based on educational resources provided by the Accredited Provider Program Director.

Please read carefully the following instructions before applying for the Contact Hours:

Contact hours may be obtained by any chapter affiliated with National Associated of Indian Nurses of America (NAINA)

8 – 12 weeks prior to the Activity:
Initial discussion of the activity 

  • Inform the Accredited Provider Program Director
  • Download the forms from the website: https://nainausa.org/
  • Accredited Provider Planning Template
  • Standard of Integrity Form  – for all  planning committee members, speaker and content experts if the program is clinical in nature.
  • Evaluation Survey for Contact Hour

6 – 8 weeks prior to the activity. 
Complete highlighted areas of the above forms and approved by the nurse planner Flyer cannot be posted until all above forms are approved and contact hour determined.

Flyers needs to have the ANCC requirements:

  • ANCC Statement 
  • NAINA logo and ANCC logo

The Accreditation marks are legally trademarked symbols and must be used in accordance with these rules: All lettering inside the logo must be readable when the symbol is used on materials. If the words are not legible, please increase the size of the logo.

The logo cannot be altered or modified. It may be resized, provided the proportions of the entire logo are maintained and all features of the logo are clearly distinguishable.

The logo shall be reproduced:

  • In black or in purple (PMS 2577 purple or equivalent)
  • In a size which makes all features of the mark clearly distinguishable
  • Without distortion of its dimensions
  • Accreditation logos cannot be used in isolation from the accredited organization’s logo. All materials must contain both the accredited organization’s logo and the appropriate Accreditation program logo. Accreditation logos cannot be larger than the organization’s logo.

For the use of the accreditation marks on electronic documentation (i.e., websites), these same rules apply.

Logos used on an accredited organization’s/program’s website must include a hyperlink to the ANCC Accreditation Program main landing page at https://www.nursingworld.org/organizational-programs/accreditation/

Prior to Activity:

Download the ANCC Statement of Disclosure and update the number of contact hours the program received along with the Learning Outcome. This powerpoint needs to be presented prior to the start of the activity.

2 weeks post activity:

  • Evaluation Summation needs to be completed.

Webinar/Conference Flyer

The following statement must be added to your webinar/conference flyer and send a copy of the flyer for NAINA records.

“The National Association of Indian Nurses of America is accredited as a provider of nursing continuing professional development by the American Nurses Credentialing Center’s Commission on Accreditation.”
