The National Association of Indian Nurses of America is delighted to announce our partnership endeavor with the DAISY Foundation to honor outstanding nurses. The Disease Attacking the Immune System (DAISY) award is an award program designed to recognize the extraordinary nurses for their compassionate care. In partnership with The DAISY Foundation, NAINA will offer the NAINA-Daisy Health Equity Award to honor Chapter who demonstrate excellence in bridging healthcare disparities by providing outstanding, compassionate care. This includes working to eliminate health disparities caused by social determinants of health to include but not limited to socio-eco-nomic, demographic, cultural, and literacy levels affecting health equity. The NAINA members who uphold the organization’s mission, vision and values to demonstrate professional excellence through their compassionate service to the community are eligible for the nomination as an honoree.

Purpose:
NAINA DAISY HEALTH EQUITY INDIVIDUAL AWARD is created to identify the efforts to combat the social determinants of Health by individuals
who make a difference through their efforts and compassionate care in the following areas:
1. Health and Health Care
2. Social and Community Context
3. Education
4. Economic Stability efforts
5. Neighborhood and Built Environment
6. Award period January 2022 to July 2023

Eligibility Criteria for Nominee and Nominator: NAINA-DAISY Health Equity Chapter/Group criteria for person being nominated and the person who is completing the nomination submission includes the following:
• Nominee must be current NAINA member by being an active member of their local chapter of NAINA.
• Nominee contributions include improving health equity and providing com passionate, quality healthcare.
• Nominator may include members of NAINA, patients, family members, nurses, physicians and other members who have direct knowledge of the
contribution to support the nomination based on their experiences or observations of extraordinary compassionate care.
• Nominators are encouraged to support their nomination by providing supporting documents  to include a narrative (not  to exceed more than 150 words), documents, pictures, audio-visual media
1. Compassionate care
2. Philanthropic contributions and efforts
3. Actions to identify socio-economic and literacy needs of the underserved population.
4. Efforts to integrating social service into holistic care.
Consent:
1. Submission implies consent for submitted material to be used by NAINA
2. All entries will become the property of the NAINA and will not be returned.
3. Incomplete and/or late submissions will be deemed ineligible for judging; Falsification of documents or any records will cause disqualification.
4. Documents must be submitted as one PDF document. Separate files or zip files are not acceptable.
